Out of network model

Our providers are not contracted with insurance and are able to offer a transparent hourly rate for treatment and varied options for improving accessibility.

Things to consider:

  • Call your insurance provider at the number on the back of your card to ask about out of network benefits and process for reimbursement. Here are useful questions to ask:

*What is my out-of-network deductible? Is this deductible individual or family?

*What are my out-of-network physical therapy benefits? Do I have a co-insurance or co-pay?

*What is the process for submitting claims? Do I have to mail my claims, fax my claims, or use a portal? What is the address or fax number if necessary?

*Is a referral or prescription required?

*Is a prior authorization required? If yes, please let your provider know PRIOR TO BEGINNING CARE. Please ask about the process for obtaining this prior authorization and pass this information along to your provider.

    *Are these CPT codes covered?:

    97161, 97110, 97112, 97140, 98530

It is beneficial to obtain the name and reference number of the representative you spoke to on the phone.

  • If you have an HSA or FSA available to you, you can use your card for payment. You will enter it on your account just like you would a credit card. All receipts will be generated when billing is processed. If you need to submit a superbill for reimbursement let your provider know and one can easily be generated!

  • Each Em/Body provider has created ways to keep quality care not cost prohibitive.

Ask your provider about the programs they have created to make care accessible for you!

    *sliding scale

    *reparations scale

    *bundle discount rates

    *and more!
